Qualifications:

• Ascertain that the needs of patients are evaluated on a regular basis.
• Oversee the coordination of internal and external referrals to the agency.
• Participate in center/agency surveys (Licensure / JCAHO) and any subsequent mandatory reports.
• In order to meet favourable recruitment and retention goals, clinical partners should be coached, monitored, and precepted.
• All aspects of patient care coordination are under your supervision.
• Verify that the patient and partner assignments are accurate.
• Collaborate with other key internal partners to support the agency’s clinical and financial success.
• Check whether the patient’s recommendations are appropriate for admission.
• Ascertain that the patient’s individualized POC is created, implemented, and maintained.
• Examine the agency’s competence to meet the patients’ predicted clinical needs before approving the admission.

Skills:

• Registered Nurse (RN) licence in the state of practise that is current and unrestricted.
• A year of experience working in a home health setting is necessary.
• You must have two (2) years of recent experience as an RN.
• It is required that you hold a valid CPR certificate.
• A nursing degree from an accredited institution or university, or a graduate of an accredited/approved RN programme, is necessary.
